von Willebrand Factor Multimer
Justification
The basic subunit of von Willebrand Factor (VWF) is a protein of molecular weight 230,000 daltons. A protomer, consisting of two of these subunits, is the basic unit which forms multimers by means of inter-dimer disulfide bonds. A heterogeneous collection of these multimers ranging in size from 500,000 daltons to over 10,000,000 daltons (species of over 20 subunits) is required for normal platelet trafficking.
VWF Multimer testing should be ordered as part of a von Willebrand Disease workup for the detection and differentiation of Type 2 VWD. Specific subtypes of Type 2 VWD are characterized by the absence of the highest molecular weight multimers.
